The procedure was updated, but the team is still using the attachment from last month's email. SOP version control needs one maintained source, visible ownership and a way to remove obsolete copies from everyday use. A revision number alone cannot tell staff where the current instruction lives.

Name the authoritative location
Choose where the approved procedure and its linked templates are maintained. Explain which versions are drafts, current or withdrawn. Use stable links from the tools people already open during work, rather than distributing new attachments every time a sentence changes.
Keep useful revision information
Document owner and approval responsibility.
Current version and effective date.
What changed and why it affects the task.
Linked templates or records also affected.
How users report an unclear step.
Use approval proportionate to the consequence. Correcting a broken link and changing an acceptance rule may need different review, even though both edit the same document.
An illustrative cleanup
A client-onboarding team has three versions of an access checklist. The process owner chooses the approved version, replaces working links and clearly withdraws the others from normal use. They explain the changed acceptance check to users instead of assuming the new filename will be noticed.
Check actual use
Include linked forms and automations in the change check. A current SOP may still open an obsolete template or trigger a workflow with an old approval rule. Test the complete route from instruction to output. Where past records must be retained, distinguish historical evidence from materials available for new work rather than indiscriminately deleting old documents.

Ask a colleague to open the instruction from their usual starting point. If they reach an old copy, fix that route. Review the document when the process or tool changes, and keep historical records where genuinely needed without presenting them as current instructions.
