Every awkward case is handled somehow, but the business never sees the pattern. A process exception log records where normal instructions were insufficient, what happened and who owns the follow-up. Use it to distinguish a genuine one-off from a repeated gap worth fixing.

Define an exception for this workflow
Examples might include missing mandatory inputs, conflicting records or a request outside the agreed route. Keep ordinary cases in the normal work queue. The exception log should explain why the standard method could not complete the case, not duplicate every task update.
Capture the minimum useful detail
Case reference and date.
Step and expected route.
What prevented normal completion.
Consequence and immediate handling.
Resolution owner and decision.
Category, recurrence and proposed process action.
Limit personal or client information to what the log actually needs. Link to approved source records instead of copying sensitive material into another shared spreadsheet.
An illustrative pattern
Three onboarding cases stall because client access is requested after the kickoff date is promised. The log shows a repeated timing problem, not three unrelated reminders. The process owner can test an earlier access check and review whether that changes the exception rate.
Close cases and review causes
Keep case resolution separate from preventive improvement. A missing input may be obtained today while the underlying form change remains open. Give the operational case its completion record and the improvement its own owner and test. Otherwise closing the immediate exception can make the recurring cause disappear from view before anyone addresses it.

Give unresolved cases an owner and next check. At a regular process review, group similar exceptions and decide whether to change instructions, inputs, authority or controls. Do not force every unusual case into an automated rule; some should remain a clearly owned judgement route. Hili Consulting can help turn repeated firefighting into evidence for improvement.
