A client asks why their record changed. The team can see the final value, but not the source, proposed change or approver. Saying the AI did it does not answer the operational question.

A useful AI workflow audit trail lets an authorised person reconstruct a case. Start with the questions the team needs to answer, then define the minimum evidence. Logging everything indiscriminately can create another information-management problem.
Work backwards from an investigation
Take a realistic question: why was this enquiry assigned to this team? The record should connect the original input, applicable rule, proposed route and final decision.
A starting specification includes:
Case identifier and relevant timestamps.
Workflow and instruction version.
Source references or approved source versions.
Proposed output and checks performed.
Human decision and responsible person.
Action attempted, result and record reference.
Failure, cancellation or retry information.
These are proposed business fields. Platforms store evidence differently, and workflows need different detail. Include a decision reason where it helps a later reviewer understand the judgement.
Distinguish proposal from completion
Approval does not establish that an update succeeded. A successful connection does not prove the correct record changed. Define evidence of completion separately.
For an illustrative email workflow, preserve the approved content version and send result separately. If the draft changes after approval, the history should show whether the revision received review.
Record partial failures. A case that completed its first action and failed its second requires different handling from one that never started.
Keep the record proportionate
Avoid passwords, unnecessary personal information and entire source archives in general activity logs. Decide who can inspect the trail and how long evidence remains available with the appropriate owners.
Operational traceability does not automatically satisfy a legal requirement. Obtain relevant specialist advice where the use case requires it.
Test reconstruction
Ask someone uninvolved to reconstruct a normal case and a failure from the evidence. Note where they must guess. Those gaps are more useful than a statement that logging is enabled.
